сложившейся эпидемиологической ситуации особое внимание должно уделяться пациентам, страдающим сердечно-сосудистыми заболеваниями, так как они в большей степени подвержены тяжелому течению новой коронавирусной инфекции. Изучение аспектов приверженности лечению и качества жизни кардиологических пациентов играет важную роль в оказании данным больным квалифицированной помощи.</p></sec><sec><title> </title><p> </p></sec><sec><title>Резюме</title><p>Резюме.</p></sec><sec><title>Цель</title><p>Цель. Изучение качества жизни и приверженности лечению пациентов, обратившихся за плановой кардиологической помощью во время пандемии новой коронавирусной инфекции.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование было включено 310 пациентов, обратившихся за медицинской помощью в кардиологический диспансер в марте 2021 г. (n = 123) и в аналогичный временной промежуток 2017 г. (n = 187). Для анализа приверженности лечению использованы шкала комплаентности Мориски – Грина, модифицированная анкета для определения факторов, влияющих на приверженность, и международный опросник EuroQol EQ5D5L.</p></sec><sec><title>Результаты</title><p>Результаты. В 2021 г. частота диагностированной хронической сердечной недостаточности среди больных, обратившихся в кардиологический диспансер, была в три раза выше, чем в 2017 г., составив 67,2% (p &lt; 0,001). Однако уровень качества жизни по шкале EQ5D5L, а также самооценка здоровья пациентов по визуально-аналоговой шкале были выше в когорте больных 2021 г. (p &lt; 0,05). При анализе приверженности лечению установлено, что примерно половина опрошенных пациентов не соблюдали врачебные назначения, что соответствовало результатам, полученным в 2017 г. Установлена ассоциация степени приверженности лечению с наличием симптомов тревоги/депрессии (r = –0,24; p = 0,017). При анализе причин плохой приверженности лечению основной причиной несоблюдения врачебных рекомендаций остается забывчивость. Финансовый аспект низкой приверженности лечению существенно увеличил значимость в 2021 г. (p = 0,05).</p></sec><sec><title>Заключение</title><p>Заключение. За время пандемии COVID-19 отмечено трехкратное увеличение доли случаев хронической сердечной недостаточности среди больных, обратившихся за специализированной амбулаторной кардиологической помощью. При этом уровень качества жизни и самооценки здоровья пациентов оказался выше в когорте пациентов 2021 г. Приверженность лечению амбулаторных кардиологических больных за изученный период (2017–2021 г.) остается низкой. Во время пандемии COVID-19 важное влияние на приверженность медикаментозной терапии оказывает финансовый аспект. Установлена отрицательная корреляционная связь между наличием психоэмоциональных расстройств и степенью приверженности лечению, что актуализирует необходимость выявления пациентов с симптомами тревоги/депрессии среди кардиологических больных и оказания им специализированной психологической помощи, в том числе с целью повышения приверженности лечению.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Highlights</title><p>Highlights</p><p>In the current epidemiological situation, special attention should be paid to patients suffering from cardiovascular diseases, as they are more susceptible to the severe course of the new coronavirus infection. The study of aspects of treatment adherence and quality of life of cardiac patients plays an important role in providing these patients with qualified care.</p></sec><sec><title> </title><p> </p></sec><sec><title>Aim</title><p>Aim. Study the quality of life and adherence to treatment of patients who applied for elective cardiac care during the pandemic of a new coronavirus infection.</p></sec><sec><title>Methods</title><p>Methods. The study included 310 patients who applied to the cardiological dispensary in March 2021 (n = 123) and in the same period in 2017 (n = 187). To analyze adherence to treatment, the Morisky-Green compliance scale, a modified questionnaire to determine the factors influencing adherence, and the international questionnaire EuroQol EQ5D5L were used. Statistical processing was performed using the STATISTICA 10.0 program.</p></sec><sec><title>Results</title><p>Results. In 2021, the incidence of diagnosed CHF among patients included in the study was three times higher than in 2017 and amounted to 67.2% (p &lt; 0.001). The level of quality of life on the EQ5D5L scale, as well as self-assessment of the health of patients on the VAS scale, were higher in the 2021 cohort of patients (p &lt; 0.05). Approximately half of the patients surveyed do not comply with medical prescriptions, which is in line with the results of 2017. Forgetfulness remains the main reason for non-compliance with medical recommendations.</p></sec><sec><title>Conclusion</title><p>Conclusion. During the COVID-19 pandemic, there has been a threefold increase in the proportion of patients suffering from CHF among the studied cohort of patients. At the same time, the level of quality of life and self-esteem of health is higher during the COVID-19 pandemic. Adherence to treatment of patients during the studied period remains low. During the COVID-19 pandemic, the financial aspect has an important impact on therapy adherence. A negative correlation was established between the presence of psychoemotional disorders and the degree of adherence to treatment.
