Выполнено ранжирование этих показателей в зависимости от величины отношения шансов.</p></sec><sec><title> </title><p> </p></sec><sec><title>Резюме</title><p>Резюме</p></sec><sec><title>Цель</title><p>Цель. Оценить прогностическую значимость лабораторных параметров как факторов летального исхода у пациентов с новой коронавирусной инфекцией (COVID-19).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В проспективное исследование включены 164 пациента, госпитализированные в моноинфекционный госпиталь в период с 03.11.2020 по 15.01.2021. Диагноз COVID-19 верифицирован по результатам ПЦР на наличие РНК SARS-CoV-2 в материале из носо- и ротоглотки. Диагностика и классификация пневмонии по данным компьютерной томографии, оценка степени тяжести заболевания по шкале NEWS, а также лечение больных осуществлены в соответствии с «Временными методическими рекомендациями. Профилактика, диагностика и лечение новой коронавирусной инфекции (СОVID-19)» (версия 9 от 26.10.2020). Конечной точкой служили исходы госпитализации: выписка или летальный исход. Первую группу состояли пациенты с летальным исходом (n = 25), вторую группу – выжившие больные (n = 139). За летальные исходы принимали общую и сердечно-сосудистую смертность.</p></sec><sec><title>Результаты</title><p>Результаты. Госпитальная летальность составила 15,2%. Мужчины составили 56% умерших, женщины – 44%. Возраст умерших – 72 [68; 83] года, выживших – 65 [59,0; 70,5] лет, p &lt; 0,001. Большинство (97,6%) пациентов находились в ясном сознании, 2,4% – в состоянии оглушенности и сопора. Предикторами летального исхода при COVID-19 явились повышенная концентрация тропонина в крови (отношение шансов (ОШ) 18,38; 95% доверительный интервал (ДИ) 6,503–51,97; p &lt; 0,001), уровень D-димера &gt; 700 нг/мл (ОШ 17,79; 95% ДИ 6,362–49,77; p &lt; 0,001), прокальцитонин &gt; 0,28 нг/мл (ОШ 7,357; 95% ДИ 2,840–19,06; p &lt; 0,001), тромбоциты &lt; 180 × 109/л (ОШ 4,76; 95% ДИ 2,00–11,1; p = 0,001). Протективным фактором определен уровень моноцитов ≥ 6,0% (ОШ 0,205; 95% ДИ 0,067–0,627).</p></sec><sec><title>Заключение</title><p>Заключение. Наиболее значимыми показателями, позволяющими оценить прогноз при COVID-19, являются тропонин, D-димер, прокальцитонин и тромбоциты. Мониторирование этих параметров позволит клиницистам заблаговременно определять пациентов с высоким риском развития летального исхода, требующих более активных лечебных мероприятий.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Highlights</title><p>Highlights</p><p>The analysis of laboratory parameters capable of assessing the lethal outcome in patients with COVID-19 was carried out. After that, these parameters were ranked depending on the magnitude of the odds ratio.</p></sec><sec><title> </title><p> </p></sec><sec><title>Abstract</title><p>Abstract</p></sec><sec><title>Aim</title><p>Aim. To assess the predictive value of laboratory parameters as factors of lethal outcome in patients with novel coronavirus infection (COVID-19).</p></sec><sec><title>Methods</title><p>Methods. The prospective study included 164 patients hospitalized in a monoinfection hospital in the period from November 3, 2020 to January 15, 2021. The diagnosis of COVID-19 was verified by PCR testing for the presence of SARS-CoV-2 RNA in nasopharyngeal and oropharyngeal specimens. Diagnosis, classification of pneumonia based on CT findings, disease severity according to the National Early Warning Score (NEWS), and treatment were conducted according to the “Temporary Guidelines. Prevention, diagnosis and treatment of a new coronavirus infection (COVID-19)”, version 9 (10/26/2020). The end of the study was the outcome of hospitalization: discharge or death. The main group consisted of 25 patients with a fatal outcome (Group 1), the 2nd group (n = 139) consisted of surviving patients.</p></sec><sec><title>Results</title><p>Results. The mortality rate was 15.2%. 56% of the deceased were males, and 44% were females. The average age of the deceased was 72 years [68; 83], compared to 65 years [59,0; 70,5], р &lt; 0,001 in the surviving group. 97.6% of patients were in clear consciousness, while 2.4% were in a state of stupor and soporific. Odds ratio (OR) analysis showed that predictors of fatal outcome in COVID-19 were elevated troponin concentration in blood (OR = 18,38 (95% confidence interval (CI) 6,503–51,97; p &lt; 0,001), D-dimer level &gt; 700 ng/mL (OR = 17,79; 95% CI 6,362–49,77; p &lt; 0,001), procalcitonin (PCT) &gt; 0.28 ng/mL (OR = 7,357; 95% CI 2,840–19,06; p &lt; 0,001), and platelets &lt; 180 × 109/L (OR = 4,76; 95% CI 2,00–11,1; p = 0,001). Monocytes ≥ 6.0% (OR = 0,205; 95% CI 0,067–0,627) were identified as a protective factor.</p></sec><sec><title>Conclusion</title><p>Conclusion. The most significant parameters for predicting prognosis in COVID-19 are troponin, D-dimer, PCT, and platelets.
