положения</p></sec><sec><title> </title><p> </p></sec><sec><title>Цель</title><p>Цель. Оценка прогностического потенциала комбинации кардиоспецифичных биомаркеров и J-волны на ЭКГ в отношении сердечно-сосудистых осложнений и долгосрочной смертности после выписки пациентов с пневмонией, ассоциированной с COVID-19.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Сывороточные биомаркеры sST2, VCAM-1 и hsTnI оценены при поступлении у 254 пациентов, госпитализированных с COVID-19-ассоциированной пневмонией. Клинические и лабораторные характеристики больных, а также концентрация биомаркеров и наличие J-волн на ЭКГ проанализированы как факторы риска сердечно-сосудистых событий через 12 мес. после выписки. </p></sec><sec><title>Результаты</title><p>Результаты. Высокий уровень VCAM-1 явился предиктором риска развития комбинированной конечной точки (инфаркт миокарда, инсульт, тромбоэмболия легочной артерии и внезапная смерть) – отношение рисков 1,049, 95% доверительный интервал 1,016–1,083. Высокий уровень sST2 выступил значимым предиктором риска как смерти (отношение рисков 1,006, 95% доверительный интервал 1,004–1,007), так и комбинированной конечной точки (отношение рисков 1,005, 95% доверительный интервал 1,003–1,006). J-волна на ЭКГ во время госпитализации оценена как статистически незначимый показатель (p &gt; 0,05) при прогнозировании внезапной смерти в период до года после выписки. </p></sec><sec><title>Заключение</title><p>Заключение. У госпитализированных пациентов, перенесших COVID-19, повышенные уровни hsTnI и sST2 при поступлении служат сильными предикторами сердечно-сосудистых осложнений во время долгосрочного наблюдения. С другой стороны, значимость J-волны на ЭКГ в прогнозировании отдаленных неблагоприятных сердечно-сосудистых событий остается пограничной.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Highlights</title><p>Highlights</p></sec><sec><title> </title><p> </p></sec><sec><title>Aim</title><p>Aim. To evaluate the prognostic potential of a combination of cardiac-specific biomarkers and ECG J-waves regarding long-term mortality and cardiovascular complications after discharge in patients with COVID-19-associated pneumonia.</p></sec><sec><title>Methods</title><p>Methods. Serum biomarkers sST2, VCAM-1 and hs-TnI were assessed at admission in 254 patients hospitalized with COVID-19-associated pneumonia. ECGs on admission were analyzed for the presence of J waves. Clinical and laboratory characteristics of patients, as well as biomarker concentrations and the presence of J-waves on the ECG, were correlated with mortality and cardiovascular complications after discharge at 12 months [FU: 347 (310, 449) days]. Cox proportional hazards models were used to identify predictors of the risk of mortality and the composite endpoint up to one year after discharge.</p></sec><sec><title>Results</title><p>Results. A high level of VCAM-1 was a significant predictor of the risk of a combined endpoint (MI, stroke, pulmonary embolism, sudden death): HR = 1.049, CI 95%: 1.016–1.083; and high sST2 level was a significant predictor of the risk of both death (HR = 1.006, CI 95%: 1.004–1.007) and the composite endpoint (HR = 1.005, CI 95%: 1.003–1.006). In the generalized multivariate model, when other influencing factors were combined, the presence of a J-wave on the ECG during hospitalization was statistically insignificant (p &gt; 0.5) for predicting sudden death up to a year after discharge.</p></sec><sec><title>Conclusion</title><p>Conclusion. In hospitalized COVID-19 survivors, elevated hsTnI and sST2 levels on admission were strong predictors of CV events during long-term follow-up.
