Оценить преимущества реваскуляризации миокарда на работающем сердце у пациентов с ишемической болезнью сердца и сочетанным поражением митрального клапана.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включены 50 пациентов с ишемической болезнью сердца и сочетанным поражением митрального клапана, которым проведены одномоментная реваскуляризации миокарда и коррекция порока митрального клапана. Пациенты разделены на две группы: в 1-й группе коронарное шунтирование проведено на остановленном сердце (n = 26), во 2-й группе реваскуляризации миокарда выполнена на работающем сердце (n = 24).</p></sec><sec><title>Результаты</title><p>Результаты. В группе реваскуляризации миокарда на работающем сердце отмечены меньшее время пережатия аорты (85,5 [71,25; 105,25] против 119,0 [99,25; 132,25] мин в группе с применением искусственного кровообращения, p &lt; 0,05), меньшая длительность искусственного кровообращения (136,5 [119,25; 158,50] против 168,5 [142,75; 186,25] мин, p &lt; 0,05) и общая продолжительность операции (292,5 [252,5; 360,0] против 340,0 [287,5; 385,0] мин соответственно, p = 0,15). Кроме того, в этой группе определена меньшая потребность в трансфузии крови и ее компонентов: свежезамороженной плазмы (2 [2; 3] против 3 [3; 3], p &lt; 0,05), эритроцитарной массы (2 [1; 2] против 2 [2; 2] соотвественно, p = 0,4), а также наблюдалась тенденция уменьшения койко-дня в стационаре (20,0 [13,50; 26,25] против 23,5 [17,5; 26,0] сут, p &lt; 0,05).</p></sec><sec><title>Заключение</title><p>Заключение. Реваскуляризация миокарда на работающем сердце у пациентов с сочетанным поражением митрального клапана является безопасной и воспроизводимой методикой, а также обеспечивает снижение времени пережатия аорты, продолжительности искусственного кровообращения и операции, уменьшает потребность в гемотрансфузии и длительность пребывания в стационаре.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Highlights</title><p>Highlights</p></sec><sec><title> </title><p> </p></sec><sec><title>Abstract</title><p>Abstract</p></sec><sec><title>Aim</title><p>Aim. To evaluate the benefits of off-pump coronary artery bypass grafting (OPCABG) in patients with coronary artery disease and concomitant mitral valve (MV) regurgitation.</p></sec><sec><title>Methods</title><p>Methods. The study included 50 patients with coronary artery disease and concomitant MV disease who underwent simultaneous CABG and correction of MV regurgitation. Patients were divided into 2 groups: group 1 (n = 26) included patiens with on-pump CABG (ONCABG), group 2 (n = 24) included patiens with OPCABG.</p></sec><sec><title>Results</title><p>Results. In the OPCABG group there was shorter aortic cross-clamping time (85,5 [71,25; 105,25] vs 119 [99,25; 132,25] min, compared with the ONCABG group, p &lt; 0,05), shorter duration of CPB (136,5 [119,25; 158,5] vs 168,5 [142,75; 186,25] min, p &lt; 0,05), and overall duration of the operation (292,5 [252,5; 360] vs 340 [287,5; 385] min, respectively, p = 0,15). Moreover, in this group there was a lower need for transfusion of blood and its components: freshly frozen plasma (2 [2; 3] vs 3 [3; 3], p &lt; 0,05), RBC mass (2 [1; 2] vs 2 [2; 2],  respectively, p = 0,4),  and  lower number of bed-days during hospital stay (20 [13,5; 26,25] vs 23,5 [17,5; 26] days, p &lt; 0,05).</p></sec><sec><title>Conclusions</title><p>Conclusions. Off-pump CABG in patients with combined valvular pathology is a safe and reproducible technique that provides shorter aortic cross-clamping time, on-pump time and operation duration.
