Изучить влияние комбинированной липидснижающей терапии (КЛТ) на параметры липидного и минерального гомеостаза у пациентов с острым коронарным синдромом (ОКС) и стабильной ишемической болезнью сердца (ИБС) на фоне дислипидемии.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование были включены пациенты с ОКС (n = 10) и стабильной ИБС (n = 5), имевшие при первичном обследовании уровень холестерина липопротеинов низкой плотности (ХС-ЛПНП) &gt; 3,5 ммоль/л. Пациенты находились под наблюдением в течение 4 мес. с ежемесячным контролем липидного профиля. КЛТ включала пероральный прием ингибиторов 3-гидрокси-3-метилглутарил-КоА-редуктазы аторвастатина (40–80 мг/сут) или розувастатина (20–40 мг/сут), ингибитора абсорбции холестерина эзетимиба (10 мг/сут) и подкожные инъекции ингибитора PCSK9 алирокумаба (300 мг/мл один раз в месяц). До начала КЛТ и после ее завершения оценены концентрация кальципротеиновых частиц (КПЧ) в сыворотке крови и склонность сыворотки крови к их формированию в условиях минерального стресса.</p></sec><sec><title>Результаты</title><p>Результаты. Пациенты с ОКС и стабильной ИБС были сопоставимы по молярной концентрации общего холестерина (ОХ), ХС-ЛПНП и триглицеридов (ТГ) до и после КЛТ. Молярная концентрация холестерина липопротеинов высокой плотности (ХС-ЛПВП) у пациентов с ОКС была ниже, чем у пациентов со стабильной ИБС (p = 0,018 и p = 0,037 до и после КЛТ соответственно). В группе пациентов с ОКС зафиксировано снижение молярной концентрации ОХ, ХС-ЛПНП и ТГ (p = 0,002, p = 0,002 и p = 0,019 соответственно), а также повышение молярной концентрации ХС-ЛПВП (p = 0,018) после лечения. До начала лечения концентрация КПЧ у пациентов с ОКС была выше, чем у больных стабильной ИБС (p = 0,04). Показатели склонности сыворотки крови к формированию КПЧ при минеральном стрессе в обеих группах существенно не различались.</p></sec><sec><title>Заключение</title><p>Заключение. Четырехмесячная КЛТ у пациентов с ОКС и стабильной ИБС продемонстрировала безопасность и эффективность в снижении ОХ, ХС-ЛПНП и ТГ. Значимого влияния КЛТ на концентрацию КПЧ и минеральную емкость сыворотки крови не отмечено. Для достижения целевых значений липидограммы требуется продолжение КЛТ с акцентом на применение эзетимиба и повышение приверженности пациентов к КЛТ с учетом необходимости применения нескольких препаратов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Highlights</title><p>Highlights</p></sec><sec><title> </title><p> </p></sec><sec><title>Abstract</title><p>Abstract</p></sec><sec><title>Aim</title><p>Aim. To evaluate the impact of combination lipid-lowering therapy (CLT) on parameters of lipid and mineral profile in patients with dyslipidemia and acute coronary syndrome (ACS) or chronic coronary syndrome (CCS).</p></sec><sec><title>Methods</title><p>Methods. We consecutively enrolled patients with ACS (n = 10) and CCS (n = 5) with low-density lipoprotein cholesterol (LDL-C) &gt; 3,5 mmol/L. The follow-up was 4 months with monthly lipid profile checkpoints. CLT included HMG-CoA reductase inhibitors (atorvastatin 40–80 mg/day or rosuvastatin 20–40 mg/day), cholesterol absorption inhibitor ezetimibe (10 mg/day), and PCSK9 inhibitor alirocumab (300 mg/mL subcutaneously, once per month). Before and after the CLT, we have also evaluated the serum level of calciprotein particles (CPPs) and serum calcification propensity.</p></sec><sec><title>Results</title><p>Results. Patients with ACS and CCS had similar total cholesterol (TC), LDL-C, and triglycerides (TG) before and after the CLT. The level of high-density lipoprotein cholesterol (HDL-C) in patients with ACS was lower than in CCS (р = 0.018 and p = 0.037 before and after the CLT, respectively). CLT reduced TH, LDL-C, and TG (p = 0.002, p = 0.002, and p = 0.019, respectively) and increased HDL-C (р = 0.018) in patients with ACS. Serum level of calciprotein particles (CPPs) in patients with ACS was higher in comparison with CCS before the treatment (р = 0.04). Serum calcification propensity did not differ between the patients with ACS or CCS.</p></sec><sec><title>Conclusion</title><p>Conclusion. CLT demonstrated safety and efficacy in patients with ACS and CCS, reducing the levels of TH, LDL-C, and TG, while not affecting serum CPP level and serum calcification propensity.
