Гипертоническая болезнь сопровождается вовлечением в патологический процесс органов-мишеней, в том числе головного мозга, приводя к развитию острых и хронических цереброваскулярных заболеваний. Современные разработки в сфере прогнозирования вариантов развития и течения ангиоцеребральной патологии могут быть использованы при создании новых методик ведения больных, находящихся в группе риска.</p></sec><sec><title>Цель</title><p>Цель. Используя возможности нейросетевого анализа определить ранжированный вклад биомаркеров нейровоспаления и белков семейства фактора некроза опухолей в патогенез нейровоспаления на фоне течения гипертонической болезни.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследовании приняли участие 80 человек. Контрольную группу составили здоровые лица. В первую группу вошли пациенты с диагнозом гипертоническая болезнь II–III стадии, достигнутым целевым уровнем артериального давления, 3–4 степенью риска развития сердечно-сосудистых осложнений. 2 группу составили пациенты с установленным диагнозом гипертоническая болезнь II–III стадии, недостигнутым целевым уровнем артериального давления, 3–4 степенью риска развития сердечно-сосудистых осложнений. В 3 группу были отобраны пациенты с верифицированным диагнозом ишемический (атеротромботический) инсульт. Полученные в результате исследования данные были использованы для обучения многослойного перцептрона и создания математической модели, определяющей ранжированный вклад биомаркеров нейровоспаления и белков семейства фактора некроза опухолей в патогенез нейровоспаления на фоне течения гипертонической болезни.</p></sec><sec><title>Результаты</title><p>Результаты. По результатам проведенного мультиплексного анализа и статистической обработки данных были выявлены схожие сдвиги нейроиммунного статуса как у пациентов с развившимся ишемическим инсультом, так и у лиц с гипертонической болезнью. Полученные результаты позволили разработать высоко достоверную нейросетевую модель для оценки вклада некоторых биомаркеров в патогенез нейровоспаления при гипертонической болезни.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные в результате проведенного исследования данные свидетельствуют об изменении показателей нейроиммунного статуса у пациентов с гипертонической болезнью. Обращает на себя внимание, что содержание биомаркеров, связанных с нейровоспалением, у пациентов с недостигнутым целевым уровнем артериального давления максимально приближено к показателям пациентов с ишемическим инсультом. В то же время в группе пациентов с достигнутым целевым уровнем АД также отмечается сдвиг исследуемых показателей, но в меньшей степени, что может указывать на наличие нейровоспалительного процесса, который присутствует даже на фоне приема антигипертензивной терапии и контроле АД. Разработанная при помощи нейросетевого анализа математическая модель позволила определить ранжированный вклад исследуемых параметров в патогенез нейровоспаления у пациентов с гипертонической болезнью. Полученные результаты в дальнейшем могут быть использованы для более персонифицированного подхода в лечении пациентов через воздействие на те или иные звенья патогенеза, тем самым модерируя протекающий процесс и оказывая влияние на дальнейший прогноз заболевания.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Highlights</title><p>Highlights</p></sec><sec><title> </title><p> </p></sec><sec><title>Abstract</title><p>Abstract</p></sec><sec><title>Background</title><p>Background. Hypertensive disease is accompanied by involvement of target organs, including the brain, in the pathological process, leading to the development of acute and chronic cerebrovascular diseases. The development of modern methods for predicting the development and course of angio-cerebral pathology can be used to create new methods for managing patients at risk.</p></sec><sec><title>Aim</title><p>Aim. Using the capabilities of neural network analysis, we determined the ranked contribution of biomarkers of neuroinflammation and proteins of the tumor necrosis factor family to the pathogenesis of neuroinflammation in the context of hypertension.</p></sec><sec><title>Methods</title><p>Methods. The study involved 80 participants. The control group consisted of healthy individuals. The first group included patients with stage II–III hypertension, achieved target blood pressure levels, and a 3–4 risk of developing cardiovascular complications. The second group consisted of patients with stage II–III hypertension, unachieved target blood pressure levels, and a 3–4 risk of developing cardiovascular complications. Group 3 included patients with a verified diagnosis of ischemic (atherothrombotic) stroke. The data obtained from the study were used to train a multilayer perceptron and create a mathematical model that determines the ranked contribution of biomarkers of neuroinflammation and proteins of the tumor necrosis factor family to the pathogenesis of neuroinflammation in the context of hypertension.</p></sec><sec><title>Results</title><p>Results. According to the results of the conducted multiplex analysis and statistical data processing, similar shifts in the neuroimmune status were identified in both patients with ischemic stroke and individuals with hypertension. Modern developments in the field of predicting the development and course of angiocerebral pathology can be used to create new methods for managing patients at risk.</p></sec><sec><title>Conclusion</title><p>Conclusion. The data obtained as a result of the conducted study indicate changes in the neuroimmune status of patients with hypertension. It is noteworthy that the content of biomarkers associated with neuroinflammation in patients with unattained target blood pressure levels is as close as possible to that of patients with ischemic stroke. At the same time, in the group of patients with achieved target blood pressure levels, there is also a shift in the studied indicators, but to a lesser extent, which may indicate the presence of a neuroinflammatory process that is present even in the presence of antihypertensive therapy and blood pressure control. The mathematical model developed using neural network analysis allowed us to determine the ranked contribution of the studied parameters to the pathogenesis of neuroinflammation in patients with hypertension.
