Многие пациенты с сердечной недостаточностью в конечной стадии умирают из-за нехватки донорских сердец. Полностью искусственное сердце (ПИС) – имплантируемый аппарат, заменяющий сердце. К настоящему времени успешно имплантировано более чем 2000 пациентам в качестве моста к трансплантации сердца. Однако, после нескольких десятилетий исследований до сих пор не создан ПИС, пригодный для целевой терапии. Высокий уровень осложнений, громоздкие устройства, низкая долговечность, плохая биосовместимость и низкое качество жизни пациентов – вот некоторые из основных недостатков существующих устройств ПИС, которые необходимо устранить, прежде чем использовать их в качестве целевой терапии. Быстро появляющиеся инновации в области аккумуляторных технологий, беспроводной передачи энергии, биосовместимых материалов и мягкой робототехники открывают многообещающие возможности для разработки ПИС и могут помочь устранить недостатки существующих устройств. В этом обзоре описываются основные вехи в истории создания и развития ПИС. Описываются различия в механизмах работы этих устройств, обсуждаются текущие проблемы и требования к следующему поколению ПИС. Таким образом, несмотря на значительный прогресс, современные ПИС остаются преимущественно временным решением. Ключевыми задачами для перехода к долгосрочной целевой терапии являются повышение долговечности устройств, решение вопроса о необходимости пульсирующего потока и миниатюризация для широкого круга пациентов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Highlights</title><p>Highlights</p></sec><sec><title> </title><p> </p></sec><sec><title>Abstract</title><p>Abstract</p><p>Cardiovascular disease is one of the leading causes of death worldwide. Many patients with end-stage heart failure die due to a shortage of donor hearts. A total artificial heart (TAH) is an implantable device that replaces the heart. To date, it has been successfully implanted in more than 2,000 patients as a bridge to heart transplantation. However, after several decades of research, a TAH suitable for targeted therapy has yet to be developed. High complication rates, bulky devices, poor durability, poor biocompatibility and poor quality of life for patients are some of the major drawbacks of existing TAH devices that need to be addressed before they can be used as targeted therapy. Rapidly emerging innovations in battery technology, wireless energy transfer, biocompatible materials, and soft robotics offer promising opportunities for TAH development and may help address the shortcomings of existing devices [<xref ref-type="bibr" rid="cit1">1</xref>]. This review describes the major milestones in the history of TAH design and development. The differences in the mechanisms of operation of these devices are described, and current challenges and requirements for the next generation of TAHs are discussed. Thus, despite significant progress, modern TAHs remain predominantly a temporary solution.
