положения</p></sec><sec><title> </title><p> </p></sec><sec><title>Цель</title><p>Цель. Оценить взаимосвязь гемореологических показателей с наличием и выраженностью атеросклероза коронарных артерий.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование было включено 303 пациента, которым выполнялась плановая коронарография (КАГ). Средний возраст составил 65,5 ± 9,5 года, 55% были мужчины. Вязкость артериальной крови, забранной непосредственно из интродьюсера до проведения КАГ, измеряли с использованием ротационного вискозиметра Brookfield DV2TLV (США) в одинаковых стабильных температурных условиях. Измерения проводились при скоростях сдвига 1, 5, 10, 22,5, 45, 90 и 225 с-1. Коэффициенты жесткости и неньютоновости рассчитывались из степенного закона вязкости жидкостей.  Оценку коронарного атеросклероза выполняли по данным КАГ с расчетом балла по шкале SYNTAX. Для устранения дисбаланса между группами пациентов применяли сопоставление индексов склонности (propensity score matching, PSM) методом ближайшего соседа 1:1 с каллипером 0,05, при пороге баланса SMD &lt; 0,2.</p></sec><sec><title>Результаты</title><p>Результаты. В исследование было включены 303 пациента. Основную группу составили 105 пациентов (34,6%) с выявленными атеросклеротическими поражениями (SYNTAX &gt; 0). Группу сравнения составили 198 пациентов (65,4%) без ангиографических признаков атеросклероза, либо с гемодинамически незначимым поражением коронарных артерий (SYNTAX = 0). Гемореологические показатели, включая вязкость крови на различных скоростях сдвига, коэффициенты жесткости и неньютоновости, не различались между группами как до, так и после PSM (p &gt; 0,05 для всех показателей). Корреляционный анализ по Спирмену не выявил статистически значимой связи между баллом SYNTAX и гемореологическими параметрами крови (r &lt; 0,1, p &gt; 0,05).</p></sec><sec><title>Заключение</title><p>Заключение. В проведенном исследовании не было выявлено статистически значимой ассоциации между гемореологическими показателями артериальной крови и распространенностью или тяжестью атеросклеротических поражений коронарных артерий. Полученные данные позволяют предположить, что неблагоприятное влияние повышенной вязкости крови на клинические исходы у пациентов с ишемической болезнью сердца обусловлено не прямым усилением атеросклеротического процесса, а иными патофизиологическими механизмами.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Highlights</title><p>Highlights</p></sec><sec><title> </title><p> </p></sec><sec><title>Aim</title><p>Aim. To evaluate the relationship between hemorheological parameters and the presence and severity of coronary atherosclerosis.</p></sec><sec><title>Methods</title><p>Methods. The study included 303 patients undergoing elective coronary angiography (CAG). The mean age was 65.5 ± 9.5 years; 55% were male. Arterial blood viscosity, sampled directly from the introducer sheath prior to CAG, was measured using a Brookfield DV2TLV (USA) rotational viscometer under standardized and controlled temperature conditions. Measurements were performed at shear rates of 1, 5, 10, 22.5, 45, 90, and 225 s⁻¹. Stiffness and non-Newtonian coefficients were calculated using the power-law model of fluid viscosity. Coronary atherosclerosis was assessed by CAG with subsequent calculation of the SYNTAX score. To minimize imbalance between patient groups, propensity score matching (PSM) was applied using the nearest-neighbor 1:1 method with a caliper of 0.05 and a balance threshold of SMD &lt; 0.2.</p></sec><sec><title>Results</title><p>Results. A total of 303 patients were included in the study. The main group consisted of 105 patients (34.6%) with angiographically confirmed atherosclerotic lesions (SYNTAX &gt; 0). The comparison group comprised 198 patients (65.4%) with no angiographic evidence of atherosclerosis or with hemodynamically insignificant coronary artery lesions (SYNTAX = 0). Hemorheological parameters, including blood viscosity at various shear rates, stiffness, and non-Newtonian coefficients, did not differ significantly between groups either before or after PSM (p &gt; 0.05 for all parameters). Spearman correlation analysis revealed no statistically significant association between SYNTAX score and hemorheological parameters (r &lt; 0.1, p &gt; 0.05).</p></sec><sec><title>Conclusion</title><p>Conclusion. This study demonstrated no statistically significant association between arterial blood hemorheological parameters and the prevalence or severity of coronary atherosclerosis.
