Определить факторы, влияющие на риск возникновения ВСС у пациентов в условиях стационара.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведён ретроспективный анализ 150 историй болезни на базе Краевого государственного бюджетного учреждения здравоохранения «Краевая клиническая больница» города Красноярска за период с 2018 по 2023 г. Из них зарегистрировано 75 (50,0%) случаев смерти от ВСС, среди которых 42 мужчины и 33 женщины в возрасте от 35 до 82 лет. Остальные 75 (50,0%) пациентов с эквивалентной половозрастной структурой, проходивших стационарное лечение в том же медицинском учреждении и выписанных из него составили группу контроля. Проанализировано более 100 показателей, включающих в себя социальные характеристики пациентов, особенности госпитализации, степени тяжести состояния, сопутствующей патологии, динамику лабораторно-инструментальных показателей.</p></sec><sec><title>Результат</title><p>Результат. По результатам однофакторного анализа Кокса установлено, что наибольший вклад в формирование риска внезапной сердечной смерти (ВСС) вносили увеличение времени от начала заболевания до момента госпитализации, наличие ишемической болезни сердца (ИБС), хронической сердечной недостаточности, артериальной гипертензии, тахикардии по данным ЭКГ, а также наличие инвалидности. При многофакторном регрессионном анализе Кокса выявлены три независимых предиктора, достоверно ассоциированные с повышением риска ВСС: увеличение времени до госпитализации (HR = 1,48; 95% ДИ 1,23–1,78), наличие инвалидности (HR = 1,83; 95% ДИ 1,09–3,07) и ИБС (HR = 2,02; 95% ДИ 1,13–3,64). Полученная модель продемонстрировала высокие прогностические характеристики (C-index = 0,74; ROC-AUC = 0,91; Se = 0,84; Sp = 0,82), что указывает на её клиническую ценность при стратификации риска ВСС у госпитализированных пациентов.</p></sec><sec><title>Заключение</title><p>Заключение. Своевременное обращение пациентов с факторами риска возникновения ВСС за медицинской помощью способно снизить их летальность. Многие предикторы ВСС можно выявить на этапе сбора анамнеза, лабораторно-инструментальной диагностики. На этапе оказания стационарной помощи разработана программа, способная на основании наличия факторов риска ВСС рассчитать риск возникновения летального исхода.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Highlights</title><p>Highlights </p></sec><sec><title> </title><p> </p></sec><sec><title>Aim</title><p>Aim. To identify factors influencing the risk of SCD in hospitalized patients.</p></sec><sec><title>Methods</title><p>Methods. A retrospective analysis of 150 medical records was conducted at the Krasnoyarsk Regional Clinical Hospital from 2018 to 2023. Among them, 75 cases (50,0%) of in-hospital sudden cardiac death (SCD) were identified, including 42 men and 33 women aged 35 to 82 years. The remaining 75 patients (50,0%) with an equivalent age and sex distribution who underwent inpatient treatment at the same institution and were subsequently discharged formed the control group. Over 100 parameters were analyzed, including patient social characteristics, hospitalization characteristics, severity of condition, comorbidities, and changes in laboratory and instrumental parameters.</p></sec><sec><title>Results</title><p>Results. According to the results of the univariate Cox regression analysis, the greatest contribution to the risk of sudden cardiac death (SCD) was associated with an increased time from disease onset to hospitalization, the presence of coronary heart disease (CHD), chronic heart failure, arterial hypertension, ECG-detected tachycardia, and disability status. In the multivariate Cox regression model, three independent predictors were significantly associated with an increased risk of SCD: longer time to hospitalization (HR = 1.48; 95% CI 1.23–1.78), presence of disability (HR = 1.83; 95% CI 1.09–3.07), and CHD (HR = 2.02; 95% CI 1.13–3.64). The constructed model demonstrated strong prognostic performance (C-index = 0.74; ROC-AUC = 0.91; Se = 0.84; Sp = 0.82), indicating its clinical utility for risk stratification of SCD in hospitalized patients.</p></sec><sec><title>Conclusion</title><p>Conclusion. Timely medical attention for patients with risk factors for SCD can reduce their mortality. Many SCD predictors can be identified through medical history collection and laboratory and instrumental diagnostics.
