В основе ремоделирования миокарда лежат изменения, контролируемые системой матриксных металлопротеиназ (Matrix metalloproteinases, MMPs) и их тканевых ингибиторов (Tissue inЫbitors of metalloproteinases, TIMPs)</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оценка динамикироточных концентраций ММР-3,-9, TIMP-1,-2 и их</p><p>связи с инструментальными показателями ремоделирования миокарда после изолированного протезирования митрального клапана (МК).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Пациентам со стенозом  (n=24) и недостаточностью (n=24) МК перед операцией и через год после протезирования МК определяли сывороточные концентрации MMP-3,-9, TIMP-1,-2 методом твердофазного иммуноферментного анализа.</p></sec><sec><title>Результаты</title><p>Результаты. Исходно отмечались повышенные уровни MMP-9, коррелирующие с фракцией выброса левого желудочка и интегральным систолическим индексом ремоделирования. Через год после операции произошло снижение концентрации ММР-9 в группе с недостаточностью МК на 18,54%, со стенозом -  на 17,17 % (р&lt;0,05), более выраженное при значительном увеличении толерантности к физической нагрузке.</p></sec><sec><title>Заключение</title><p>Заключение. У пациентов с изолированными митральными пороками активация MMP-9 тесно ассоциирована со структурным ремоделированием камер сердца и выраженностью изменений толерантности к физической нагрузке.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. The matrix metalloproteinase (MMP) system and their tissue inhibitors (TIMPs) act to control myocardial remodeling processes.</p></sec><sec><title>Aim</title><p>Aim. To evaluate serum levels of MMP-3, -9, TIMP-1,     -2 and their relationships with instrumental parameters of myocardial remodeling after isolated mitral valve replacement.</p></sec><sec><title>Methods</title><p>Methods. All patients enrolled into the mitral stenosis group (n = 24) and the mitral insufficiency group (n=24) underwent measurements of serum levels of MMP-3, -9, TIMP-1, -2 by solid-phase enzyme immunoassay before mitral valve replacement and 1 year after it.</p></sec><sec><title>Results</title><p>Results. Initial elevated levels of MMP-9 correlated with the left ventricular ejection fraction and the integrated systolic remodeling index. One year after mitral valve replacement, MMP-9 levels decreased by 18.54% in the mitral insufficiency group and by 17.17% in the mitral stenosis group (p -0.05). The decrease was more pronounced with a significant increase in exercise tolerance.</p></sec><sec><title>Conclusion</title><p>Conclusion.
