На основании данных одноцентрового регистрового исследования была проведена оценка отдаленных результатов у 228 больных со стабильным течением ИБС, подвергшихся неполной реваскуляризации при помощи коронарного шунтирования (КШ) с использованием анастомоза ВГА к ПНА.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Все больные были распределены в 2 группы. Первую группу составили пациенты с остаточным (после КШ) SYNTAX score ≤8 (группа Ост. SYNTAX ≤8, n = 75), вторую – с остаточным SYNTAX score &gt;8 (группа Ост. SYNTAX ≥9, n = 153).</p></sec><sec><title>Результаты</title><p>Результаты. Причинами неполной реваскуляризации были: (1) малый диаметр дистальной части пораженных коронарных артерий у 52 (23%) пациентов, (2) кальциноз целевых артерий у больных и трансмуральные рубцовые изменения миокарда в зоне кровоснабжения заинтересованной артерии у 18 (8%) пациентов, (3) тяжелая сопутствующая патология у 33 (14%) больных, (4) опасность и/или невозможность наложения проксимального анастомоза с восходящей «no touch aorta» в 35 (16%) случаях, (5) гипоплазия коронарных артерий у 11 (5%) больных, (6) непригодность графтов для шунтирования задней и боковой стенок миокарда у 6 (2%) пациентов, (7) технические сложности визуализации коронарных артерий на задней и боковой стенках, обусловленные последствиями синдрома Дресслера у 9 (4%) больных. (8) В 64 (28%) случаях неполная реваскуляризация признавалась целесообразной по комплексу причин. К концу трехлетнего срока наблюдения количество случаев смерти от любых причин (3 (4%) в группе Ост. SYNTAX ≤8 против 6 (3,9%) в группе Ост. SYNTAX ≥9, Р = 0,7) и кардиальной смерти (2 (2,7%) в группе Ост. SYNTAX ≤8 против 5 (3,3%) в группе Ост. SYNTAX ≥9, Р = 0,9) были сопоставимы в анализируемых группах. Потребность в повторной реваскуляризации коронарных артерий в группах была на протяжении наблюдаемого периода одинаковой (Р = 0,4). По числу случаев комбинированной конечной точки группы не отличались (6 (8%) в группе Ост. SYNTAX ≤8 против 10 (6,5%) в группе Ост. SYNTAX ≥9, Р = 0,9).</p></sec><sec><title>Заключение</title><p>Заключение. Таким образом, в исследовании не было выявлено связи между неполной реваскуляризацией миокарда посредством КШ (ВГА к ПНА) и уровнем остаточного SYNTAX score. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To estimate long-terms results of incomplete revascularization with the internal thoracic artery to the left anterior descending anastomosis performed during mammary-coronary bypass grafting (MCBG) in 228 patients with stable coronary artery disease using the single-center registry database.</p></sec><sec><title>Methods</title><p>Methods. All patients were enrolled into 2 groups. Group 1 consisted of patients with residual SYNTAX score ≤8 (group SYNTAX ≤8, n = 75), Group 2 with residual SYNTAX score &gt;8 (group SYNTAX ≥9, n = 153).</p></sec><sec><title>Results</title><p>Results. The reasons for the incomplete revascularization were: (1) small diameter of the distal segment of the coronary arteries in 52 (23%) patients, (2) calcification and transmural scar of the myocardium in 18 (8%) patients, (3) severe concomitant pathology in 33 (14%) patients, (4) danger and / or inability to place proximal anastomosis on the ascending aorta in 35 (16%) cases, (5) coronary artery hypoplasia in 11 (5%) patients, (6) unfit grafts for shunting in 6 (2%) patients, (7) technical difficulties in visualization of coronary arteries on the posterior and lateral walls due to Dressler's syndrome in 9 (4%) patients. (8) Incomplete revascularization was considered reasonable in 64 (28%) cases due to the number of causes. The three-year all-cause mortality was 4% in the group of SYNTAX ≤8 versus 3.9% in the SYNTAX group ≥9, P = 0.7, similarly to the cardiac death – 2.7% in the group of SYNTAX ≤8 versus 3.3% in the group SYNTAX ≥9, P = 0.9. The need for repeat coronary revascularization was the same in both groups (P = 0.4). The groups did not differ in the number of the combined endpoint (8% in the group of SYNTAX ≤8 versus 6.5% in the group SYNTAX ≥9, (P = 0.9).</p></sec><sec><title>Conclusion</title><p>Conclusion.
