В обзорной статье представлены современные данные, характеризующие проблему консервативного ведения пациента с острым коронарным синдромом (ОКС) разного типа. Учитывая высокую частоту отказа по тем или иным причинам от инвазивного лечения у пациентов с ОКС, проблема оптимизации консервативного ведения больных является достаточно актуальной. Авторами были представлены современные эпидемиологические данные по проблеме, выделены категории пациентов, которые чаще всего не направляются на инвазивное лечение – прежде всего, это лица пожилого возраста, больные с многососудистым поражением и вовлечением дистального русла, пациенты с наличием хронической болезни почек. Авторами был составлен обобщенный профиль пациента, для которого вероятнее будет избрана консервативная стратегия ведения, а также рассмотрена оптимальная медикаментозная терапия у пациентов в зависимости от вида ОКС.

The review article updates current data concerning drug therapy in patients with acute coronary syndrome (ACS) of different types. High refusal rate to invasive management among patients with ACS in our country suggests that the need for optimizing drug therapy. Recent epidemiological data are presented in addition to the main target groups of patients who do not receive invasive treatment: elderly patients, patients with multivessel coronary artery disease and distal lesions and those who have chronic kidney disease. The general patient profile who will be prescribed drug therapy is presented. The optimal drug therapy regimens for patients with different ACS types are provided.
