Конечными точками исследования на протяжении госпитального периода были такие неблагоприятные кардиоваскулярные события, как смерть, инфаркт миокарда, острое нарушение мозгового кровообращения (ОНМК) / транзиторная ишемическая атака (ТИА), кровотечения, связанные с КШ, повторная незапланированная реваскуляризация миокарда. Полнота проведенной реваскуляризации оценивалась при помощи подсчета остаточного (резидуального) SYNTAX Score. Также учитывался объем периоперационной кровопотери и длительность пребывания пациентов в отделении реанимации и клинике. Регистрировались раневые осложнения, нарушения ритма и проводимости, осложнения со стороны дыхательной системы.</p></sec><sec><title>Результаты</title><p>Результаты. Средний возраст пациентов составил 60±8,9 (36–82) лет. Средний показатель хирургического риска по шкале EuroScore II в исследуемой выборке – 1,61±1,66%, тогда как выраженность коронарного атеросклероза по шкале SYNTAX – 11,7±9,5 (5–25,5) баллов. После шунтирования резидуальный SYNTAX составил 1,8±2,1(0– 12,5). Подавляющее большинство пациентов имели однососудистое поражение коронарного русла (n = 108, 73,9%). Более чем у половины пациентов отмечен постинфарктный кардиосклероз. Сахарный диабет диагностирован у 17,8% больных. Ожирением I-II степени страдали 19% больных. Среднее значение фракции выброса левого желудочка в общей выборке пациентов соответствовало значению 61,3±7,5% (37–74%). При осуществлении хирургического доступа в восьми случаях (5,5%) потребовалась конверсия боковой миниторакотомии в стернотомию и шунтирование ПНА на работающем сердце. Средний объем интраоперационной кровопотери составил 293,4±117,9 мл. Неполная целесообразная реваскуляризация миокарда была достигнута в 24,7% случаев. В структуре неблагоприятных кардиоваскулярных событий не зарегистрировано случаев госпитальной летальности. Встречаемость таких осложнений, как инфаркт миокарда, ОНМК/ТИА и повторная незапланированная реваскуляризация миокарда, находилась в допустимом диапазоне значений – 0,7%, 0% и 2,7% соответственно. В 6,1% (n = 9) случаях послеоперационный период осложнился развитием раневого осложнения в виде диастаза кожи и подкожно-жировой клетчатки. Среднее время пребывания пациентов в отделении реанимации составило 1±0,2 сутки. Абсолютное большинство пациентов (n = 108) выписались из отделения кардиохирургии в срок 10±1,2 суток.</p></sec><sec><title>Заключение</title><p>Заключение. В госпитальном периоде наблюдения стратегия реваскуляризации off-pump КШ ПНА из минидоступа продемонстрировала удовлетворительные результаты, что выражалось в незначительном количестве кардиоваскулярных осложнений и объеме периоперционной кровопотери, отсутствием глубокой раневой инфекции, коротким госпитальным периодом. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate the in-hospital results of minimally invasive direct coronary artery bypass grafting (MIDCAB) of the left anterior descending artery (LAD) on the beating heart.</p></sec><sec><title>Methods</title><p>Methods. 146 patients with stable coronary artery disease and hemodynamically significant LAD lesions were included in a single-center prospective study. The study endpoints included death, myocardial infarction (MI), stroke, bleedings associated with the CABG, repeated non-elective myocardial revascularization during the in-hospital period. The completeness of the performed revascularization was assessed by estimating the residual SYNTAX score. Additionally, the volume of perioperative blood loss and patients’ ICU and in-hospital length of stay were recorded. Wound complications, heart rhythm and conduction disturbances, respiratory complications were assessed.</p></sec><sec><title>Results</title><p>Results. The mean patient age was 60±8.9 (36–82) years. The mean EuroScore II was 1.61±1.66%. The SYNTAX score was 11.7±9.5 (5–25.5) scores. After MIDCAB, the residual SYNTAX was 1.8±2.1 (0–12.5). The majority of patients had single-vessel disease (n = 108; 73.9%). More than half of the patients had postinfarction cardiosclerosis (PICS). 17.8% of patients had diabetes mellitus. 19% of patients were present with class 1–2 obesity. The mean left ventricular ejection fraction (LVEF) in the total sample was 61.3±7.5% (37–74%). Eight patients (5.5%) required the conversion of lateral minitracotomy to sternotomy followed by the LAD grafting on the beating heart. The mean volume of intraoperative blood loss was 293.4±117.9 mL. Reasonable incomplete myocardial revascularization was achieved in 24.7% of cases. There were no cases of in-hospital deaths in the study cohort. The incidence of MI, stroke and repeated non-elective myocardial revascularization was within the acceptable range of 0.7%, 0% and 2.7%, respectively. 6.1% of patients (n = 9) had wound complications (deep incisional surgical site infection). The mean length of stay in the intensive care unit was 1±0.2 days.
