Пациенты были рандомизированы в две группы: (1) Чрескожное коронарное вмешательство (ЧКВ) с имплантацией биодеградируемого стента «Absorb» (n = 65) и (2) Малоинвазивное маммаро-коронарное шунтирование («MIDCAB») (n = 65). Группы были сопоставимы по всем исходным клинико-демографическим и ангиографическим характеристикам. Среднее значение по шкале SYNTAX и EuroScore II составило – 7,46±2,12 и 0,83±0,3% соответственно. Первичной конечной точкой исследования были значимые неблагоприятные кардиоваскулярные и цереброваскулярные события (MACCE) на протяжении 3 лет наблюдения. Также анализировали дисфункцию целевого сосуда/шунта (ДЦС) – по клиническим и ангиографическим критериям в течении 3 лет после вмешательства.</p></sec><sec><title>Результаты</title><p>Результаты. Анализ 3-х летних результатов показал отсутствие достоверных различий между ЧКВ и MIDCAB по частоте общей летальности, ИМ как в целом, так и в целевом сосуде и ОНМК. В тоже время пациенты, перенесшие ЧКВ с имплантацией биодеградируемого каркаса, достоверно чаще нуждались в повторной реваскуляризации по сравнению с группой MIDCAB (13,8% и 3,1%; р = 0,027). Однако, достоверной разницы между группами по частоте МАССЕ не выявлено (ЧКВ 16,9%, MIDCAB 9,2%, р = 0,19). Несмотря на отсутствие различий по отдельным показателям, таких как кардиальная смерть и ИМ, в совокупности ЧКВ значимо уступает MIDCAB по встречаемости неблагоприятных событий, связанных с имплантированным устройством (12,3% и 3,1%, соответственно; р = 0,04).</p></sec><sec><title>Заключение</title><p>Заключение. ЧКВ с имплантацией биодеградируемого сосудистого каркаса и малоинвазивное маммаро-коронарное шунтирование в группе пациентов с изолированным поражением ПНА показали сопоставимые отдаленные результаты по частоте неблагоприятных кардиоваскулярных событий. Однако, ЧКВ с имплантацией биодеградируемого каркаса в данной когорте пациентов ассоциируется</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. Evaluates 3-year clinical outcomes of percutaneous coronary intervention (PCI) with bioresorbable vascular scaffold (BVS) versus minimally invasive direct coronary artery bypass (MIDCAB) surgery for the treatment of left anterior descending (LAD) lesions.</p></sec><sec><title>Methods</title><p>Methods. In this single-center study were included 130 patients with stable angina and significant (≥70%) LAD disease. Patients were randomly assigned in a 1:1 ratio to PCI with everolimus-eluting bioresorbable vascular scaffold (Absorb) (n = 65) or MIDCAB (n = 65). The primary end-point was major adverse cerebrocardiovascular events (MACCE) and secondary was. Primary and secondary endpoints were, respectively, major adverse cerebro-cardiovascular events (MACCE) and target vessel failure at 3-year.</p></sec><sec><title>Results</title><p>Results.The groups of patients were comparable for all baseline demographic, clinical and angiographic parameters. The primary composite endpoint of MACCE through 3 years occurred in 16.9% of BVS patients and 9.2% of MIDCAB patients (p = 0.19). But 3-year any revascularization rates were higher with BVS (13.8% vs. 3.1%; p = 0.027). TVF was favor of the MIDCAB group (12,3% vs. 3,1%, p = 0.04), mainly triggered by high subsequent need for revascularization of the targeted vessel in the BVS group (9.2% vs. 1.5%; p = 0.05).</p></sec><sec><title>Conclusion</title><p>Conclusion. At 3-year follow-up, PCI by BVS and MIDCAB in in patients with isolated LAD lesions yielded similar long-term outcomes regarding the primary composite clinical endpoint.
