Необходимо ли всем пациентам выполнять коронароангиографию для дифференциальной диагностики инфаркта миокарда 1-го и 2-го типа?

Аннотация

В дискуссионной статье рассмотрен вопрос о целесообразности проведения коронароангиографии (КАГ) всем пациентам для дифференциальной диагностики инфаркта миокарда 1-го и 2-го типа. С точки зрения оказания пациенту помощи - решение должно приниматься индивидуально с учетом соотношения пользы/риска. В основе обоснованного решения должен лежать принцип улучшения состояния пациента, облегчение симптомов заболевания, снижения риска смерти. В настоящее время недостаточно данных Резюме               для точного определения каким именно пациентам проведение КАГ определенно необходимо в качестве этапа решения вопроса о тактике ведения, поэтому требуется проведение дальнейших исследований. До получения надежных доказательств необходима согласованная позиция специалистов по обоснованию необходимости/целесообразности проведения КАГ в особых клинических ситуациях. Инвазивные исследования не должны выполняться ради уточнения диагноза с целью статистического учета и для исключения расхождения клинического и патологоанатомического диагноза.
